Senior Care and eldercare services Campbell River bcOur simple, five-step approach to eldercare planning:

1. Free Consultation

Every relationship begins with a free, 30-minute phone consultation. This is a great opportunity to discuss your concerns about your aging relative, ask any questions about the process and discover how we may be able to ease your anxiety by coordinating the care of your loved one.

After this initial consultation, we’re generally able to provide an estimate for services that may be required.

Call 250-650-2359 to get started!

2. Discovery Consultation

Ideally done at the home of your aging relative, this consultation forms the backbone of your personalized senior care-service plan. We provide a comprehensive evaluation of your loved one’s well-being – including quality of life, daily challenges or limitations, formal and informal support systems and psychological and social functioning.

In the case of long-distance caregivers, or in cases where a meeting with the aging relative isn’t easily facilitated, we will schedule a consultation with the caregivers themselves, either in person or by phone.

3. Personalized Senior Care-Service Plan

From the information we gathered during your discovery consultation, we summarize current issues and areas for future discussion and prepare a series of recommendations for the care of your aging relative. We take into consideration the amount of care that family or friends are able to provide, and to what extent other health or senior care providers will assist with the caregiving.

We encourage and enable family members to manage the care of their aging relative as much as they choose or are able. At the end of this process you’ll walk away with a step-by-step action plan and the resources to help you implement it.

4. Implementation

If the family desires, we research available care and service options and their associated costs. The extent to which we participate in arranging care depends entirely on the availability of other caregivers, such as friends, family and other health-care providers. Our role is simply to ensure that your loved one gets the timely care that he or she needs, with an absolute minimum of stress and anxiety.

5. Reporting

We provide you with regular updates on the care of your aging relative, including any new challenges and successes. We can even provide you with a customized care organizer that will give you quick access to critical details, including health and medical information, important legal and financial documents and notes from third-party caregivers.

For families separated by geography, we can create your own family web site, where every family member will have access to the latest information and updates on the aging relative’s care.

We’re here to help you care for your aging relative, but it’s up to you to take the first step.

Call 250-650-2359 today for a free 30-minute consultation.
